Purpose

The aim is to tell about a patient who had a history of surgery for myopia correction and who had suffered several attacks of herpetic keratitis. He underwent keratotomy, which did not prevent him from undergoing cataract surgery in the future. As a result, visual acuity was 0.9. He was also diagnosed with open-angle glaucoma in both eyes in 2019

Setting

Patient A. a 62 -year-old man, presented with decreased vision in both eyes. His medical history revealed that at age 20, he underwent surgery to correct high myopia and myopic astigmatism in both eyes. Radial and tangential corneal incisions were made in both eyes. In 2019, he suffered three attacks of recurrent herpetic keratitis in his left eye. He has a hereditary history of glaucoma and, based on perimetry and OCT, was diagnosed with primary open-angle glaucoma  in both eyes

Methods

In 2026, phacoemulsification with intraocular lens implantation was performed on the left eye.

IOL was calculated using the Barrett True K formula with the desired postoperative correction to emmetropia. The operation was performed using a standard technique with scleral incisions on Stellaris PC ( Bausch& Lomb).

Results

The result of the operation was visual acuity - 0.9 with mild myopic astigmatism in the left eye. Іmplanted lens - 23,50 D Alcon SN60WF. The patient continues to receive drug therapy for glaucoma, further monitoring of the patient is carried out. Phacoemulsification with intraocular lens implantation is planned for the right eye.

Conclusions

Cataract surgery in patients who have undergone keratotomy is a complex task that requires careful calculation of IOL. Patients with recurrent herpetic keratitis in preoperative preparation for cataract surgery need prevention of this disease. Diagnosis of open-angle glaucoma and subsequent monitoring of its progression requires further standardization of research methods. There is no objective method for measuring IOP in such patients, which complicates further diagnosis and monitoring of them
