Purpose

to present long term results of ranibizumab treatment in eyes with neovascular glaucoma already treated with combined cataract surgery and Ahmed valve implantation

Setting

Department of Ophthalmology, General Hospital of Lamia, Lamia, GREECE

Methods

case series of 2 male patients  (3 eyes) with neovascular glaucoma (NVG) and cataract. Patient 1, age 70 , diabetic NVG, Patient 2 , age 65, NVG in both eyes after Branch Retinal Artery Occlusion. Initial IOPs  were 38, 50 and 25 mmHg, respectively. All eyes had combined Ahmed valve implantation and cataract surgery (phaco and three pieces intraocular lens (IOL) )  and subsequently Ranibizumab (Lucentis) injections approximately every 3 months. Follow up was 6, 24 and 36 months respectively.

Results

Intraocular pressure remained below 21mmHg with topical medications during the follow up period and visual acuity was stable. IOL remained well positioned. However, all patients presented temporary IOP rise in case of exacerbation of neovascularization. Treatment with ranibizumab restored hypertonia.

Conclusions

the combination of Ahmed valve implantation, cataract surgery and antiVEGF injection on a PRN scheme (approximately every 3 months) provides a reliable treatment regimen for neovascular glaucoma. Closed follow up and patient consultation is necessary for the successful treatment of these patients.
