Purpose

To evaluate the long-term safety of the Asqelio intraocular lens (IOL) by assessing the incidence of Nd:YAG capsulotomy within five years after cataract surgery.

Setting

Oftalvist clinics in Alicante, Madrid, Valencia, and Granada, Spain.

Methods

This multicenter retrospective observational study included patients reviewed during 2025 at participating centers. Eyes that underwent phacoemulsification with Asqelio IOL implantation were analyzed. Surgery dates for right and left eyes were recorded. Nd:YAG capsulotomy was documented as a binary outcome (yes/no), with the corresponding date when applicable. The primary endpoint was the occurrence of Nd:YAG capsulotomy within 5 years postoperatively. Time-to-event analysis was performed using Kaplan-Meier survival estimates. Eyes without Nd:YAG capsulotomy were censored at 5 years after surgery. Administrative censoring was applied at December 31, 2025. Follow-up duration was summarized with censoring at 5 years.

Results

A total of 148 patients (256 eyes) were included. Mean age at surgery was 74.7±8.4 years. During the follow-up period, 20 eyes (7.8%) required Nd:YAG capsulotomy, while 236 eyes (92.2%) were censored without experiencing the event. Kaplan-Meier analysis demonstrated a cumulative Nd:YAG capsulotomy rate of approximately 8% at five years. The mean time free from Nd:YAG capsulotomy was 4.82 years (95% CI: 4.74–4.91). The median time to Nd:YAG could not be estimated because fewer than 50% of eyes experienced the event during follow-up.

Conclusions

In this multicenter cohort, implantation of the Asqelio intraocular lens was associated with a low five-year Nd:YAG capsulotomy rate, with more than 90% of eyes remaining free from laser treatment at the end of follow-up. These findings support a favorable long-term safety profile of the Asqelio IOL regarding posterior capsule opacification requiring Nd:YAG capsulotomy.
