Purpose
To describe the EASI-IOL (Extraocular Assembled Scleral-sutured Iris-IOL) technique: a two-suture, four-point ePTFE scleral fixation method for implanting a preassembled prosthetic iris–intraocular lens (IOL) complex in aphakic eyes with aniridia.
Setting
Retrospective case series at tertiary ophthalmology centers in Beirut, Lebanon.
Methods
Six eyes of five patients underwent pars plana vitrectomy followed by implantation of a preassembled artificial iris–IOL construct. The prosthetic iris was assembled extraocularly to a four-haptic IOL. Two CV-8 ePTFE sutures were passed through opposing haptics (180° apart) to create four-point fixation and were secured to the sclera to achieve stable centration while minimizing intraocular manipulation.
Results
All eyes achieved stable centration. Photophobia and glare improved in all patients. Best-corrected visual acuity improved in five eyes and was limited in one by ocular comorbidities. No suture erosion, postoperative intraocular pressure elevation, or reoperation occurred during follow-up. Mild posterior tilt occurred in one case but did not compromise the visual axis.
Conclusions
Extraocular preassembly with two-suture, four-point ePTFE scleral fixation provides a simplified approach for artificial iris–IOL implantation in selected aniridic aphakic eyes, with good centration and early safety in this small series.
