Purpose

To evaluate early experience at distance, intermediate and near, with the TECNIS PureSeeTM, a refractive presbyopia-correcting IOL, in a real world cataract population with a range of mild ocular comorbidities.

Setting

Refractive cataract surgery private practice, Melbourne, Victoria.

Methods

Retrospective case series. Consecutive cataract patients with mild ocular comorbidities underwent uncomplicated cataract surgery, single surgeon, with implantation of the TECNIS PureSeeTM IOL (toric or spherical). Biometry was performed with the Zeiss IOLMaster700, using the Barrett Universal-II(TK) formula. The post-operative refractive target was -0.20D to -0.30D for sphere, with the lowest refractive astigmatism. Review at 4 weeks post-operatively. Uncorrected visual acuity was assessed at distance, intermediate (66cm) and near (33cm) with the Precision-Vision ‘Near or Intermediate’ chart. Subjective refraction and best distance corrected visual acuity was assessed.

Results

26 eyes of 16 patients were assessed. Mild ocular comorbidities included ERM, early AMD with small drusen, EBMD, glaucoma suspect and forme-fruste keratoconus. Mean age (±SD) was 69.9(±11.7) years, 87.5% female. Mean refractive prediction error (predicted–actual spherical equivalent) was -0.12D(± 0.39D). Mean UCDVA (logMAR) was 0.02(± 0.08). BCDVA 0.00(± 0.06) with a mean spherical equivalent refractive error of -0.12D(± 0.41D). Mean UCIVA (intermediate) was 0.21(± 0.11) (6/10 equivalent). Mean UCNVA (near) was 0.38(± 0.14) (6/15 equivalent).

Conclusions

Patients with mild ocular comorbidities undergoing cataract surgery and implantation of the
TECNIS PureSeeTM IOL achieved excellent uncorrected distance visual acuity with low refractive error post-operatively. Useful intermediate and near acuity was achieved. This presbyopia-correcting IOL can be considered in eyes with mild ocular disease.
