Purpose

To compare objective and subjective visual outcomes after bilateral implantation of two non-diffractive extended depth of focus (EDOF) intraocular lenses (IOLs): AcrySof® IQ Vivity™ (Alcon) and ELON® (Medicontur).

Setting

Retrospective, multicenter comparative study at Rotschild Foundation and COSC (Centre Ophtalmologie Saint Cloud), France

Methods

Thirty-four patients (68 eyes) underwent bilateral phacoemulsification and implantation with either Vivity or Elon EDOF IOLs. Visual acuity (distance, intermediate, near), contrast sensitivity (CSV-1000), defocus curves, aberrometry (OPD-Scan III), photic phenomena (halo & glare simulator), and patient-reported outcomes (Catquest-9SF) were assessed 3 months postoperatively.

Results

No statistically significant difference was found in uncorrected or corrected visual acuity at any distance (p > 0.05). Defocus curves were comparable, both showing extended performance from 0.00 D to –2D with a CDVA> 0.2 LogMar (Figures 1, 2) but a slightly better intermediate vision with the Vivity. Both are insuffisant for near vision with a total spectacle independance . Vivity provided better monocular contrast sensitivity at low spatial frequencies (3 and 6 cpd, p < 0.05), but not binocularly (Tables 1, 2). Aberrometry profiles (RMS and HOAs) were similar. Both IOLs showed mild photic phenomena; Vivity had slightly more halos, though non-significant. Catquest-9SF responses showed high satisfaction in both groups.

Conclusions

Vivity and Elon EDOF IOLs deliver equivalent and effective restoration of distance and intermediate vision with minimal photic disturbance and high satisfaction. Both are appropriate choices for patients desiring refractive cataract surgery with reduced spectacle dependence.
