Purpose
To compare visual performance, contrast sensitivity, stereopsis, and patient-reported outcomes following implantation of a refractive extended depth-of-field (EDF) intraocular lens using either a mini-monovision or emmetropia target strategy.
Setting
Prospective, randomized, controlled, patient- and examiner-masked clinical trial.
Methods
Sixty patients (120 eyes) undergoing bilateral cataract surgery were implanted with a refractive EDF intraocular lens (DEN00V, Tecnis PureSee). Patients were randomized to receive either binocular mini-monovision (target refraction −0.75 D in the non-dominant eye) or bilateral emmetropia. Three months postoperatively, binocular uncorrected and corrected distance visual acuity (UDVA/CDVA), uncorrected intermediate (UIVA) and near visual acuity (UNVA), low-contrast visual acuity, defocus curves, stereopsis, and patient-reported outcomes (PRSIQ, AIOLIS) were assessed.
Results
Binocular UDVA and CDVA were excellent in both groups, with no statistically significant differences. UIVA and UNVA demonstrated a non-significant trend favoring the mini-monovision group. Binocular defocus curves were largely comparable between groups, with a slight advantage for the emmetropia group at +1.0 D. Stereopsis was preserved in both groups without significant intergroup differences. Patient-reported outcomes indicated high overall satisfaction and low spectacle dependence for distance and intermediate vision in both groups. The mini-monovision group reported slightly lower near spectacle dependence but a marginally higher incidence of glare phenomena.
Conclusions
Bilateral implantation of the DEN00V refractive EDF intraocular lens provides excellent distance and intermediate visual outcomes with good contrast sensitivity and preserved stereopsis, irrespective of targeting strategy. A mini-monovision approach offers a functional benefit for near vision without clinically meaningful compromise in distance vision or subjective visual quality.
