Purpose

In a prospective study, corneal aberrations and surgically induced astigmatism (SIA) were examined in cataract surgery after clear corneal incision (CCI) using femtosecond laser versus manual access.

Setting

A prospective study, randomized, single-site at the ORASIS Clinic and Swiss Research Foundation in Reinach, Switzerland.

Methods

Seventy-eight cataract patients were randomly divided into two groups: 38 eyes underwent femtosecond laser-assisted cataract surgery (FLACS), while 40 eyes underwent conventional manual cataract surgery (CCS). Preoperative and six-week postoperative SIA, HOA, and all topographic and refractive data were analysed for both FLACS and CCS.

Results

The CCIs generated by FLACS proved to be equivalent to the manual CCIs. The mean SIA was 0.44 ± 0.27 dioptres (D) in the FLACS group and 0.58 ± 0.46 D in the CCS group (p = 0.18), with less variability in the FLACS group. The root mean square (RMS) of corneal HOA after six weeks was 0.69 ± 0.17 µm in the FLACS group and 0.80 ± 0.56 µm in the CCS group (p > 0.05). These results confirm the efficacy, reproducibility and safety of FLACS.

Conclusions

Although not statistically significant, FLACS resulted in a numerically lower SIA and lower variability than manual CCIs. Both groups were comparable in terms of HOA, although higher mean values and higher variability were observed in the CCS group.
