Purpose

Phakic intraocular lenses (IOL) have been used in patients for refractive correction where LVC (LASER vision correction) is not possible. The phakic IOL can be angle-fixated, iris-fixated or sulcus-fixated. The sulcus-fixated phakic IOL has been widely used for refractive surgery in patients who are not suited for LVC. The Implantable Phakic Contact Lens (IPCL V2.0) is a single-piece posterior chamber phakic IOL. It can be inserted through a 2.8 mm sub-incision and is made of Axially Oriented Hydrophilic Acrylic Copolymer material. This study was done to assess the long-term efficacy and safety of the Implantable Phakic Contact Lens (IPCL V2.0) for refractive correction in patients with myopia and myopic astigmatism not amenable to LVC.

Setting

This is a single-center prospective interventional study that was done in a tertiary eye care center in eastern India. The study period was from January 2019 to December 2025. All the surgeries were performed by a single surgeon.

Methods

This prospective interventional study included patients who presented to our hospital for refractive correction during the study period. Patients were screened for refractive surgeries, and patients not eligible for the LVC but suitable for phakic IOL were enrolled in this study. IPCL V2.0 was implanted in all the patients and were followed up for 6 months. Patient data were collected, including pre-operative and post-operative refractive error and best corrected visual acuity (BCVA), pre-operative anterior chamber depth
(ACD), white to white diameter (WTW), post-operative intraocular pressure (IOP) at 1-week, post-operative vault at 1 week, 1 month, and 6 months. Statistical analysis was done using appropriate tools.

Results

A total of 200 eyes of 103 patients were enrolled with the mean age of 29.25±6.13 years (39 males and 64 females). Pre-operative spherical equivalent for OD was -11.0±3.97 D, and -10.93±3.91 D for OS. The range of spherical error was from -0.5 to –20.0 D for OD and -1.0 to -23.0 D for OS and for cylindrical error was from 0 to 4.5 D for OD and 1.75 to 4.5 for OS. The pre-op ACD and WTW were 3.12±0.25 mm and 11.45mm, respectively. The mean post-op IOP was 15.5±2.8 mm of Hg, (range 10-22 mm hg). The post-op vault at 1 week, 1 month and 6 months for OD was 688.29±151.87µ, 721.65±191.15µ and 657±228.21µ respectively and for OS was 692.39±184.02µ, 719.27±196.17µ and 656.1±194.67µ respectively and the change was not significant (p >0.05).

Conclusions

IPCL is an effective and safe method for refractive correction in patients where LVC is not possible for any reason. In our study, the IOL was noted to be stable with no significant change in vault post-implantation. There was no major adverse event during the course of the study.
