Purpose

To evaluate the visual performance and subjective visual function of a partial-range of field (RoF) enhanced intraocular lens (IOL) using a mini-monovision strategy.

Setting

This single-center, prospective clinical trial was conducted at the Department of Ophthalmology and Optometry, Medical University of Vienna, Austria.

Methods

One hundred eyes (50 patients) scheduled for bilateral cataract surgery were included in this prospective clinical trial. The partial-RoF enhanced Vivinex Impress IOL (Hoya Surgical Optics) was implanted bilaterally. The target refraction was set to emmetropia (0D) for the dominant eye and mild myopia (-0.75D) for the non-dominant eye. Two to four months postoperatively, the manifest refractive spherical equivalent (MRSE), binocular uncorrected visual acuity, defocus curve, pupil size, IOL tilt and decentration, Titmus stereopsis, corneal HOAs, and subjective visual function (VF-7) were analyzed.

Results

The MRSE was -0.14±0.26 D in the dominant eye and -0.84±0.44 D in the non-dominant eye. Mean binocular uncorrected visual acuity (UDVA, UIVA at 80cm, UIVA at 66cm) was ≤0.05 logMAR, while uncorrected near visual acuity (UNVA) was 0.20±0.11 logMAR. The defocus curve demonstrated a broad range of vision with ≤0.2 logMAR from  +0.50 D to -2.22 D. Photopic pupil size and corneal spherical aberrations had a significant impact on intermediate vision (p< 0.05). Corneal HOAs, IOL tilt and decentration were overall low. No patient reported bothersome photic phenomena; daily visual function (VF-7) and binocular stereopsis remained within normal limits.

Conclusions

Bilateral implantation of a partial-RoF enhanced IOL using a mini-monovision approach provided excellent visual performance across various distances. High subjective visual function and the absence of significant photic phenomena underline the clinical utility of this strategy to increase the range of vision.
