Purpose
To evaluate the visual outcomes of a monofocal intraocular lens (IOL) with enhanced intermediate function in patients with posterior segment pathologies.
Setting
Dr Rajendra Prasad Centre for Ophthalmic Sciences, All India Institute of Medical Sciences, New Delhi
Methods
This prospective interventional study included 68 eyes of 34 patients: 17 patients with moderate NPDR without macular edema and 17 patients with mild to moderate glaucoma who underwent bilateral phacoemulsification with TECNIS Eyhance IOL implantation. Outcomes assessed at 3 months postoperatively included uncorrected distance (UDVA), intermediate (UIVA), and near (UNVA) visual acuity, Strehl ratio, modulation transfer function (MTF), contrast sensitivity (CS), stereopsis, colour vision, and patient-reported satisfaction.
Results
The mean age was 65.5 ± 4.6 years. At 3 months, mean UDVA, UIVA, and UNVA (logMAR) were 0.08 ± 0.11, 0.25 ± 0.17, and 0.49 ± 0.15, with 74% achieving UIVA of 20/40 or better. Significant improvements were observed in postoperative stereopsis [Near: 57.2″ (NPDR); 67.6″ (Glaucoma)], CS [CS-Avg: 1.71 ± 0.14 (NPDR); 1.66 ± 0.13 (Glaucoma)], and color vision [TES: 61.6 ± 16.4 (NPDR); 64.5 ± 11.5 (Glaucoma)]. Glaucoma eyes showed marginally lower CS values than NPDR at higher frequencies (CS-C p=0.05; CS-D p=0.02). Strehl ratio and MTF improved significantly in NPDR (p<0.001) but not in glaucoma (p>0.4). Glare (4%) and halos (3%) were minimal, with 60% reporting spectacle independence for near.
Conclusions
Eyhance IOL provided excellent acuity and functional vision in NPDR and glaucoma. Optical quality gains were more pronounced in NPDR, but glaucoma eyes maintained good contrast sensitivity and colour vision, supporting its role even in eyes with posterior segment disease.
