Purpose

To evaluate the short-term efficacy and optical performance of the ZEISS CT LUCIA 621P IOL in cataract patients, using a diopter-stratified approach to assess whether its non-constant aspheric design mitigates power-dependent aberrations across a wide IOL power spectrum

Setting

Single center, retrospective , comparative study

Methods

This study includes 304 eyes from cataract patients with CT LUCIA 621P implantation. IOL power was calculated using the BU II formula. Eyes were grouped by IOL power: Group 1 (<20.0 D, n=49), Group 2 (20.0–21.5 D, n=137), Group 3 (22.0–23.5 D, n=60), and Group 4 (≥24.0 D, n=58). Outcomes included UDVA/CDVA, UIVA and UNVA , cumulative Snellen VA percentages, defocus curves (mono/binocular, photopic/mesopic), internal HOA (spherical and coma), contrast sensitivity at 1 month, and refractive errors, assessed at 1 ,3 months. Multivariable regression adjusted for age, sex, and pupil size. Statistical analyses employed ANOVA for initial comparisons and General Linear Model for inter-group differences and temporal changes (p<0.05).

Results

: All groups achieved excellent mean UDVA (0.10–0.21 logMAR, p=0.162),with balanced UIVA ( 0.38–0.40 logMAR, p=0.542) and UNVA ( 0.56–0.58 logMAR, p=0.577).Cumulative Snellen VA showed ≥95% of eyes reaching 20/32 or better, with Group 2 and 3 demonstrating superior stability. Defocus curves were comparable across groups, maintaining tolerance under photopic and mesopic conditions. Internal HOAs stayed low and stable (SA RMS ~0 across groups, change <0.02 RMS; coma RMS p=0.465),confirming optical consistency at 3 months.CS at 1 Mo was robust across groups (1.5 cpd: 1.25–1.37 logCS; 18 cpd: 0.40–0.54 logCS, p>0.05),with intermediate powers (Groups 2/3) trending higher. Refractive outcomes stabilized by 3 months (SE: -0.48 to -0.83 D, p=0.452).

Conclusions

The non-constant asphericity of the CT LUCIA 621P preserves visual quality across diopter ranges, with balanced efficacy and stability by 3 months, particularly in intermediate powers. This diopter-stratified analysis provides novel subgroup evidence for its utility in cataract patients, informing personalized IOL selection.
