Purpose

To review the clinical outcomes and safety profile of excimer laser trabeculostomy (ELT) in open-angle glaucoma.

Setting

ELT is a form of minimally invasive glaucoma surgery that provides an alternative treatment to traditional surgical methods, lowering intraocular pressure (IOP) with improved recovery times and reducing the need for topical medications.

Methods

Patients who received ELT were reviewed over 6 months in this retrospective study. The primary outcomes were to measure the changes in IOP, number of topical IOP-lowering medications and visual acuity from baseline.

Results

18 patients had ELT, with 22 eyes operated on in total. 21 eyes had phacoemulsification in combination with ELT. The mean IOP at baseline was 15.25 mmHg with a standard deviation (+) of 3.62 mmHg and decreased to 12.82 ± 3.67 mmHg at 3 months (P = 0.0293). Average IOP at 6 months was 14.71 ± 2.93 mmHg (P = 0.7539). The mean number of IOP-lowering eye drops decreased from 1.59 ± 0.73 mmHg to 0.86 ± 0.69 mmHg at 6 months (P = 0.0279). Visual acuity (logMAR) improved from 0.46 ± 0.50 to 0.27 ± 0.38 (P = 0.5889). Complications occurred during 9% of the procedures (n = 2). 27% of patients had post-operative complications, including cystoid macular oedema (n = 3), hyphaema (n = 1), uveitis (n = 2) and episcleritis (n = 1).

Conclusions

ELT combined with phacoemulsification decreased IOP and the number of IOP-lowering eye drops, whilst improving visual acuity. The safety profile and long-term efficacy of this intervention need to be reviewed.
