Purpose

To evaluate the feasibility, patient engagement, and early clinical impact of a comprehensive mobile phone–based ophthalmology application designed to improve appointment adherence, medication compliance, and follow-up continuity.

Setting

Single-center tertiary ophthalmology practice.

Methods

A custom-designed mobile application was developed integrating:

  • Online appointment scheduling

  • Secure upload and storage of patient documents

  • Automated eyedrop reminders

  • Follow-up visit reminders

  • Online video consultation capability

  • WhatsApp-enabled chat interface

  • Self-assessment visual testing modules

The application was deployed over a 6-month period. A total of 1,200 patients registered and actively used the platform. Engagement metrics, follow-up adherence, and medication compliance were assessed through app usage analytics and clinical follow-up records.

Results

Patient Engagement

  • 1,200 active users within 6 months

  • 78% regularly used reminder features

  • 64% utilized document upload function

  • 42% completed at least one video consultation

Appointment Adherence

  • Pre-app adherence rate: 68%

  • Post-app adherence rate: 89%

  • Absolute improvement: 21%

  • Missed appointment rates reduced from 32% to 11%.

Medication Compliance

  • Baseline estimated compliance: 61%

  • With automated eyedrop reminders: 86%

  • Relative improvement: 41%

Follow-Up Completion

  • Pre-app scheduled follow-up completion: 58%

  • Post-app completion rate: 84%

  • Absolute improvement: 26%

Self-Assessment & Triage

  • 38% used visual self-testing module at least once

  • 14% reported early symptoms leading to timely review

  • Reduced unnecessary physical visits by approximately 18%

Conclusions

Implementation of a comprehensive ophthalmology mobile application resulted in:

  • Significant improvement in appointment adherence (+21%)

  • Enhanced medication compliance (+25 absolute increase)

  • Improved follow-up completion (+26%)

  • Effective integration of teleconsultation and digital triage

The platform demonstrates a scalable and sustainable digital health model capable of improving long-term ophthalmic care compliance and operational efficiency.Longer follow-up is warranted to evaluate clinical outcome and cost-effectiveness impact.
