Purpose

To evaluate early tomographic stability and visual outcomes after wavefront-guided PRK in eyes with preoperative corneal irregularity.

Setting

Research Department at Ofttalmosalud, Instituto de Ojos, Lima, Peru.

Methods

Prospective interventional case series including 12 eyes with irregular corneas (post-refractive surgery, post-traumatic, or corneal scarring). Pre- and 1-month postoperative assessments included visual acuity, Scheimpflug tomography (Pentacam), and anterior segment OCT. Paired statistical comparisons were performed.

Results

UDVA improved from 0.96 ± 0.66 to 0.52 ± 0.60 logMAR (p=0.0088), while CDVA remained stable (0.08 ± 0.08 vs 0.17 ± 0.21; p=0.125). Anterior keratometry showed no significant changes: Kmax 46.83 ± 2.81 D vs 46.50 ± 3.52 D (p=0.48); Km 43.66 ± 2.76 D vs 43.30 ± 3.47 D (p=0.41). Central corneal thickness decreased from 518 ± 76 µm to 473 ± 76 µm (p=0.0005), and corneal volume from 59.93 ± 6.75 mm³ to 58.08 ± 6.22 mm³ (p=0.001). Anterior Q-value became more prolate (-0.41 ± 0.22 to -0.66 ± 0.54; p=0.02). BAD-D increased (3.36 ± 3.43 to 4.48 ± 3.25; p=0.001).

Conclusions

Wavefront-guided PRK in irregular corneas demonstrated significant visual improvement at one month.
