Real-World Outcomes Of The Elita Femtosecond System Versus Klex Platforms And Lasik For Refractive Error Correction

Purpose

To compare refractive outcomes, contrast sensitivity, and higher-order aberrations (HOAs) up to 30 days after Smooth Incision Lenticular Keratomileusis (SILK) using the ELITA femtosecond laser with outcomes from other KLEx platforms and LASIK in routine clinical practice.

Setting

Nobel Medical Group, Taipei, Taiwan

Methods

A total of 168 eyes from 84 patients (48 female, 36 male) with myopia (−5.57 ± 1.0 D; range −0.75 to −13.75 D) underwent refractive surgery. Procedures included SILK with the ELITA femtosecond laser (N=16), SMILE with the VisuMax 500 (N=36) and VisuMax 800 (N=60), and LASIK with the EX500 excimer laser (N=56). Contrast sensitivity (photopic, mesopic, scotopic) was measured using the CGT-2000, and HOAs were assessed with the OPD-Scan III. Evaluations were performed preoperatively and at 7 and 30 days postoperatively.

Results

At 1 week, 90% of VM500 eyes achieved 20/20 UDVA (EX500 80.6%, VM800 78.6%, ELITA 68.8%). At 1 month, 100% of ELITA eyes achieved 20/20 UDVA (VM800 96.4%, VM500 and EX500 91.7%). ELITA showed lower spherical aberration, coma, and total HOAs than other platforms (all P < 0.05). Contrast sensitivity declined transiently at 1 week across all machines but returned to baseline by 1 month with no significant inter-machine differences.

Conclusions

Although early visual recovery was slower with ELITA, by 30 days all ELITA SILK eyes achieved 20/20 UDVA. ELITA also showed lower HOAs, coma, and spherical aberration. Contrast sensitivity recovered to baseline by 1 month across all platforms with no significant inter-machine differences.
