Purpose
To evaluate the refractive, visual, aberrometric, and safety outcomes of posterior chamber phakic intraocular lens (IOL) implantation in adults with high myopia–associated amblyopia.
Setting
This prospective interventional study was conducted at a tertiary eye care center in Kolkata, India, between January 2021 and April 2024 and included 14 eyes of 12 adults with anisomyopic or isomyopic amblyopia who underwent posterior chamber phakic IOL implantation.
Methods
Preoperative and postoperative assessments included best-corrected visual acuity (BCVA), cycloplegic refraction, intraocular pressure (IOP), corneal aberrometry, endothelial cell density (ECD), anterior segment optical coherence tomography (AS-OCT) vault evaluation, and patient-reported satisfaction, with follow-up up to 1 year.
Results
The mean age was 25.3 ± 3.49 years, Mean BCVA improved from 0.39 ± 0.25 to 0.19 ± 0.21 logMAR at 6 months (p < 0.001). Mean spherical equivalent improved from −12.8 D to −0.75 D at 6 months (p < 0.001). Total higher-order aberrations decreasing from 11.60 µm to 1.57 µm which was statistically significant . Mean endothelial cell density decreased from 3131.33 ± 181.01 cells/mm² to 2981 ± 171.62 cells/mm² and Mean IOP increased from 14.2 ± 2.52 mmHg preoperatively to 15.22 ± 0.34 mmHg at 1 year (p = 0.28) but both were not statistically significant. AS-OCT demonstrated a stable postoperative vault with a mean of 296 µm. and no vision-threatening complications were observed. Mean patient satisfaction score was 4.74 ± 0.36
Conclusions
Posterior chamber phakic IOL implantation provides significant improvement in refractive error, visual acuity, and optical quality, So both quality and quantity of vision were improved after phakic IOLs implimentations in adults with high myopia–associated amblyopia which was clinically observed and statistically provrn in this study with a favorable short-term safety profile.
