Purpose

To evaluate visual and refractive outcomes and vault prediction accuracy of Eyecryl® phakic intraocular lenses for correction of myopia and myopic astigmatism.

Setting

Single−center prospective study conducted in the Refractive Surgery Unit of Unidade Local de Saúde de Santo António, Portugal.

Methods

Prospective study including 30 eyes undergoing Eyecryl® phakic IOL implantation (23 toric and 7 spherical). Vault prediction was calculated using the Biotech® online calculator including crystalline lens rise measurements. Preoperative and 1-month postoperative assessments included manifest refraction, best corrected visual acuity (BCVA) measured in logMAR and endothelial cell count (ECC). The postoperative vault was measured with Anterion®. Postoperative complications and the need for management of the lens sizing were evaluated.

Results

Mean patient age was 31.4±6.1 years and mean preoperative spherical equivalent was −8.65±5.70 D (−24.00 to −0.50 D). No eye lost lines of BCVA, with a safety index of 1.01. Mean ECC was 2573±162 cells/mm² and 2535±168 cells/mm² at month 1. No postoperative IOP spikes were observed.
At 1 month, 47% of eyes were within ±0.25 D, 90% within ±0.50 D, and 97% within ±0.75 D of predicted refraction. Mean achieved absolute refractive correction error was 0.28±0.19 D.
Postoperative vault exceeded predicted values (positive prediction error) in 93% of eyes, with a mean error of +192 µm. Vault predictability was within ±100 µm in 43%, ±200 µm in 53%, ±300 µm in 73% and ±400 µm in 93% of cases. One eye (3.3%) required IOL rotation due to excessive vault.

Conclusions

Eyecryl® phakic IOLs demonstrated good short-term safety and high refractive predictability. However, vault prediction using the Biotech® online calculator showed a consistent positive deviaion from calculated values, suggesting a tendency toward higher postoperative vault than predicted.
