Purpose

To evaluate the clinical and safety outcome of phakic Visian toric implantable collamer lens (ICL) (STAAR Surgical, USA) insertion after corneal collagen cross-linking (CXL) in mild to moderate keratoconus.

Setting

Private tertiary ophthalmology clinic / surgery center

Methods

This is a retrospective study to examine the results of the two-step approach to correct refractive error in mild to moderate stable keratoconus. CXL treatment and then 4-6 months later Visian toric ICL implantation were performed in 19 eyes of 11 patients. Data were collected preoperatively at 1-, 2-, 3- and 9-month follow-up visits after each treatment.

Results

At 9-month follow-up after ICL implantation, mean uncorrected distance visual acuity improved from 0.85 ± 0.49 to 0.17 ± 0.08 logMAR (P = .001) and mean corrected distance visual acuity improved from 0.21 ± 0.06 to 0.04 ± 0.04 logMAR (P = .018). Mean spherical equivalent decreased from -5.6 ± 3.53 to -0.15 ± 0.76 D (P = .001) and mean cylinder decreased from 3.6 ± 1.28 to 0.8 ± 0.64 D (P = .001). Final spherical and cylindrical error ranged from +0.50 to -1.50D and -0.50 to -1.25D, respectively. No intraoperative or postoperative complications occurred.

Conclusions

Implantation of the Visian toric ICL following CXL is an effective option for improving visual acuity in patients with keratoconus.
