Purpose

To evaluate the predictability of post-ICL vaulting using the LOVC formula based on absolute UBM measurements.

Setting

Bellecour Vision Clinic, France

Methods

This retrospective study included 61 eyes of 33 myopic patients who underwent ICL (STAAR) implantation, toric or non-toric, with a mean preoperative myopia of −9.41 ± 3.52 D.

Preoperatively, anterior segment biometric parameters were measured using Absolute UBM (Lumibird), including: sulcus-to-sulcus (STS), ciliary body inner diameter (CBID), STS lens rise (STSL), anterior chamber depth (ACD), iridocorneal angle, scotopic pupil diameter, and angle-to-angle (ATA).

The power of the ICL was determined using STELLA (STAAR) software.

The size of the ICL was chosen according to the LOVC formula, defined as: Vault = a*CBID + b*STSL + c*ICL POWER + d* ICL SIZE + e* SPD

Vaulting at 1 month post-operatively was measured by OCT and analysed.

Results

The predicted mean vaulting was 536 ± 141 µm and the vaulting measured at 1 month was 548 ± 191 µm, with no significant difference (p = 0.57).

Eighty per cent of eyes had vaulting between 250 and 750 µm, 3% between 100 and 250 µm, and 10% between 750 and 1000 µm. No eyes had extreme values (<100 µm or >1000 µm).

The vaulting obtained was within a margin of ±100 µm, ±200 µm, ±300 µm and ±400 µm of the target value for 49%, 94%, 97% and 100% of eyes, respectively.

Bland-Altman analysis showed satisfactory agreement between predicted and observed values, with a bias of 12 µm and 95% agreement limits of −317 to +413 µm.

Conclusions

The LOVC formula provides good predictability of post-ICL vaulting with Absolute UBM. Limitations related to the retrospective nature and sample size require confirmation by a larger prospective study.
