Purpose
To investigate the visual and refractive outcomes and night vision performance questionnaire results of the first cohort of V5 Implantable Collamer Lens (ICL) implantations for myopia correction in China.
Setting
The EVO Visian ICL (STAAR, USA) is a posterior chamber phakic lens with proven long-term efficacy for moderate to high myopia. The V5 model, featuring a larger optical diameter (5.0–6.1 mm vs. 4.9–5.8 mm in the V4c), was widely introduced in China by December 2025. Night vision disturbances (glare/halos) commonly affect satisfaction when pupils exceed the optical zone, yet V5 data in China remain scarce. This study assessed visual outcomes and night vision symptoms following V5 implantation.
Methods
A prospective and randomized study. Patients from Chengdu Aier EYE Hospital who underwent ICL V5 implantation in December 2025. The visual acuity, spherical equivalent (SE), vaults, corneal endothelial count (ECD), higher-order aberrations (HOA), contrast sensitivity frequencies (CSF), and visual quality questionnaire were collected before operation and at 1-day, 1-week, 1-month, and 3 months visit after ICL V5 implantation.
Results
A total of 22 eyes from 11 patients were enrolled. The mean SE was -7.42 ± 2.08 Dpreoperatively. Final UDVA averaged -0.095 ± 0.054 logMAR, with 100% achieving 20/20 or better. Predictability was high: 100% within ±0.25 D of target, with a mean SE of 0 ± 0.17 D. Safety and efficacy indices were both 1.03 ± 0.01. Mean vault was 508 ± 218 μm. Postoperative total RMS, HOA, and spherical aberration slightly decreased, while coma and trefoil slightly increased (p > 0.05). Contrast sensitivity decreased at low spatial frequencies (1.8–6 CPD) but increased at high frequencies (12–24 CPD). Night vision symptoms (glare/halos) gradually diminished postoperatively, particularly in patients with larger pupils, with most patients reporting satisfaction.
Conclusions
V5 Implantation of the ICL, first cohort in China, displayed high levels of efficacy, refractive predictability, and safety for correcting myopia, yielding excellent visual quality and high patient satisfaction.
