Purpose

The introduction of the SmartSight platform with the NOVA Femtosecond laser system represents a next generation refinement in lenticule extraction technology, incorporating low energy pulse delivery, enhanced centration algorithms , real time cyclotorsion conpensation called the CENTRAX technology, and optomised lenticule geometry.

The purpose of this study is to evaluate the clinical outcomes, safety profile, predictability, visual recovery and early refractive stability of the first 100 consecutive eyes undergoing SmartSight lenticule extraction using the NOVA platform in Chennai .

We aim to analyze :

Uncorrected distance visual acuity ( UDVA) outcomes

Refractive predictability ( spherical equivalent accuracy)

Stability over 3 to 6  months

Setting

This prospective observational study was conducted at a teritiary eye care and refractive surgery center in Chennai, India.

The SmartSight procedure were performed using the SCHWIND ATOS Femtosecond laser system equipped with the NOVA software platform and the CENTRAX technology.

All surgeries were performed by a single experienced refractive surgeon to minimize inter surgeon variability and ensure consistency in surgical technique.

Methods

Primary outcome measures

- UDVA at 1, 3 and 6 months

- Percentage of eyes achieving 20/20 or better

-Percentage within ±0.50 D and ±1.00 D of target

Secondary outcomes

- Stability of spherical equivalent

-Change in corrected distance visual acuity ( CDVA)

- Post opertative complications

-Dry eye symptom score

Statistical analysis was performed usind paired T-Tests .

Safety index and efficacy index were calculated in this study

Results

A total of 100 eyes ( 50 Patients) were included in the study .

Visual outcomes :

At 1 month :

- 95% achieved UDVA ≥ 20/20

- 98 % achieved UDVA ≥ 20/25

At 3 months :

- 98 % achieved UDVA ≥ 20/20

- 100 % achieved UDVA ≥ 20/25

At 6 months :

- 98 % achieved UDVA ≥ 20/20

- 100 % achieved UDVA ≥ 20/25

Safety :

- No eye lost ≥ 1 line of CDVA

- 4 eyes lost 1 line temporarily at 1 month but recovered by 3 months

- Mean spherical equivalent regression between 1 and 6 months were minimal .

-  No progressive ectatic changes were observed during follow up

Conclusions

SmartSight lenticule extraction using the NOVA platform demonstrated excellent early safety , efficacy and predictibality with rapid visual recovery in the first 100 eyes treated.

These results supports the integration of next generation lenticule technology into high volume refractive practice, with outcomes comparable to established lenticule extraction platforms.
