Purpose
A clinical case review to present the high complexity in hypermetropia surgery with Smile Pro and conclude that this king of surgery needs highly experienced surgeon.
Setting
The cases were selected from the Dr Holhos clinics in Romania. Dr Holhos clinics is a private ophthalmology network and the biggest refractive network in the country with three refractive centers using Smile Pro surgery.
Methods
We reviewed the results of investigations for hypermetropia patients before and after Small Incision Lenticule Extraction surgery, with specific investigations.
Study case for selected high complexity cases of hypermetropia performed with SMILE Pro.
The 1st case patient had interface bleeding during surgery. This decreased the visibility during surgery.

The 2nd case patient moved the eye during the laser phase of the surgery resulting in lenticule border extension up to the surface of the cornea.

Results
SMILE Pro is faster, does not require a flap, it maintains the biomechanics of the cornea, the anterior stromal part remains intact, it improves visual acuity, produces the lowest percentage of dry eye among all refractive techniques, it is pain-free during and after the intervention.
All cases were successfully finalized and the patients’ vision is also improved.
Conclusions
Small-Incision Lenticule Extraction (SMILE Pro) surgery technique is minimally invasive, offers the best quality of vision in the shortest time, the best after surgery outcome on dry eye syndrome, the best choice for athletes, pilots, patients sensitive to pain, patients who require the fastest recovery and challenging cases.
In all the cases presented, only the high experience of the surgeon could ensure the successful outcome of the surgery.
