Purpose
To evaluate safety, binocular visual performance, and refractive predictability of PresbyEDOF, an individualized corneal laser strategy designed to create functional extended depth of focus with tailored anisometropia.
Setting
Retrospective, single-surgeon case series including 114 eyes of 57 presbyopic patients treated with PresbyEDOF using the SCHWIND AMARIS 1050RS platform. The technique induces controlled spherical aberration and approximately 1.5 D anisometropia to achieve binocular extended depth of focus.
Methods
Outcomes at 3 months included binocular uncorrected distance (UDVA) and near visual acuity (UNVA), change in corrected distance visual acuity (CDVA), refractive accuracy, residual astigmatism, retreatment rate, and patient-reported visual quality.
Results
At 3 months, 92% achieved binocular UDVA of 0.0 logMAR or better. For near vision, 93% achieved ≥0.2 logMAR and 40% reached 0.0 logMAR. No patient lost more than one line of CDVA. Residual astigmatism ≤0.50 D was observed in 96% of eyes. Retreatment was required in 5.3%, primarily to optimize intermediate vision. No intraoperative complications, infections, or cases of ectasia were observed. Patient-reported visual quality was high, with rare and non-disturbing photopic phenomena.
Conclusions
PresbyEDOF demonstrated strong binocular performance across distances, high refractive predictability, and a favorable safety profile at 3 months. The ability to fine-tune refractive targets highlights the reversibility and adaptability of this corneal EDOF approach, positioning it as a minimally invasive alternative to lens-based presbyopia correction.
