Purpose
To report a case of high hyperopia and astigmatism with an anterior chamber depth (ACD) <3mm managed with the Visian implantable collamer lens (ICL) phakic intraocular lens
Setting
SOF Oftalmologia, Cordoba, Argentina
Report of case
The patient is a 21 year old man with cycloplegic refraction of +6.50 +2.00 x 49 in right eye and +7.00 +2.00 x 116 in left eye. Also in biometry he presented ACD <3mm (2.9mm in both eyes). He underwent bilateral ICL hyperopic toric implantation, No intraoperative or postoperative complications were observed during the 6 month follow-up. The manifest refraction improved to +0.75 -0.75 x 120 in right eye and +0.25 +0.50 x 79 in lef eye. The vault is > 250µm in both eyes.
Conclusion/Take home message
This case demonstrates that Visian ICL implantation may provide safe and effective refractive correction in carefully selected hyperopic patients with ACD slightly below the recommended threshold, with satisfactory vault and no short-term complications. Careful biometric evaluation and close follow-up are essential.
