Purpose
To report the outcomes of applying the non-linear aspheric profile of PRESBYOND Laser Blended Vision to pseudophakic patients to manage complex refractive scenarios where standard monotherapy is insufficient.
Setting
This report analyzes three cases of advanced refractive correction using PRESBYOND.
Report of case
Case 1 – Unilateral Correction for Early Presbyopic: A 41-year-old patient with a moderate cataract an in one eye received a monofocal IOL. The fellow eye with -6.00 diopter was treated with PRESBYOND to create a customized blend zone for seamless distance binocular vision.
Case 2 – Unilateral Correction for Late Presbyopic: A 61-year-old patient with extreme myopic anisometropia, -15.00 diopter in one eye, and -5.00 diopter in the fellow one. The management involved a multifocal IOL in extreme high myopic eye and PRESBYOND in the fellow eye to balance.
Case 3 – Bi-layered Correction: For extreme axial myopia (AL > 30mm) exceeding LASIK safety limits, a neutral-SA monofocal IOL was first implanted to address the high dioptric error. This was followed by secondary PRESBYOND to treat residual error and presbyopia.
Results: Initial observations indicate that the customized corneal ablation patterns successfully increased the range of vision from distance to near across all three surgical strategies. In Case 1 and Case 2, patients reported high satisfaction with binocular fusion and spectacle independence, successfully bridging the gap between different optical platforms, monofocal/multifocal IOLs and corneal ablation. In Case 3, the "bi-layered" approach safely bypassed anatomical corneal limits while providing the added benefit of increased depth of focus. No safety concerns regarding excessive aberration or loss of contrast sensitivity were noted at the follow-up.
Conclusion/Take home message
PRESBYOND is described as a safe and versatile method to improve vision in patients who have had IOL implants, combining lens stability with tailored corneal adjustments. While early outcomes are promising, showing high patient satisfaction and reduced need for glasses, further large-scale studies are needed to standardize these procedures
