Purpose

To assess and compare the early and long-term (up to 12 months) safety profile in patients with stage 1–3 open-angle glaucoma who underwent PreserFlo implantation.

Setting

Russia, Moscow, Doctor’s Shilova Clinic

Methods

The study included 36 patients (42 eyes) with primary open-angle glaucoma. Mean age was 64 years; 6 patients underwent bilateral implantation, and 18 eyes had repeat glaucoma surgery.
In 8 eyes the natural crystalline lens was preserved; 18 eyes were pseudophakic; and 16 eyes had phacoemulsification with immediate subsequent PreserFlo drainage implantation. Outcome measures included the level and stability of IOP throughout follow-up, visual acuity metrics, perimetry, and tomographic parameters of the optic nerve head and macula.

Risk of complications during the follow-up was evaluated: transient or persistent hypotony, hypertension, maculopathy, choroidal detachment, hemorrhagic, endothelial layer safety and inflammatory events.

Results

Following PreserFlo implantation, complete success (IOP ≤21 mmHg and ≥20% reduction without medications) was achieved in ~84% of patients; an additional ~15.5% achieved success with adjunctive medications (qualified success); and <1% (0.45%) was considered a relative failure due to the need for additional laser surgery.

In the early postoperative period, the most common findings were hypotony (~7.1%), choroidal detachment requiring posterior sclerotomy (~4.3%), and microhyphema (~9.5%). Endothelial cell loss was <0.2% over 12 months. No cases of inflammation, device protrusion/exposure, or needling were observed.

Conclusions

PreserFlo MicroShunt demonstrates a stable safety profile, provides high functional outcomes, and is an effective solution, including for combined surgery in glaucoma and complicated cataract.
