Purpose

To evaluate the effectiveness of combined phacoemulsification + IOL and iStent implantation for the reduction of intraocular pressure and glaucoma medication burden. Outcomes were compared against nationally established benchmarks of ≥22% IOP reduction and ≥1 medication reduction in at least 50% of the patients.

Setting

Single surgeon practice in South Wales with patients who underwent cataract surgery and the iStent procedure.

Methods

A retrospective audit of the first 20 consecutive eyes undergoing phakoemulsification + IOL and iStent implantation in a single surgeon's practice was performed. Inclusion criteria included a diagnosis of primary open angle glaucoma, ocular hypertension requiring topical treatment or normal tension glaucoma, as well as a minimal follow up of 2 months. We recorded preoperative and postoperative IOP, and the number of medications used. Percentage IOP reduction and proportion of patients achieving ≥1 medication reduction was calculated and compared with published benchmarks.

Results

Twenty eyes were included and the mean preoperative IOP was 17.3mmHg, which reduced to a mean postoperative IOP of 11.5mm Hg. An IOP reduction of 34% on average was seen, exceeding the national benchmark. Reduction of at least one medication was noticed in 50% of patients with several patients able to discontinue topical medication

Conclusions

Combined phakoemulsification + IOL and iStent implantation resulted in a clinically significant reduction in IOP, exceeding the national benchmark by 12%, and a reduction in glaucoma medication burden that meets the national benchmark. This audit shows that outcomes for a new adopter of iStent are comparable to the established national standards, supporting the effectiveness of iStent implantation for patients with glaucoma and ocular hypertension requiring treatment, and for new surgeons who wish to introduce iStent into their practice.
