Purpose

To determine real world changes in intraocular pressure after cataract surgery based on glaucoma diagnosis status and the presence or absence of concomitant glaucoma surgery.

Setting

Integrated Academic Medical Center – Mayo Clinic - USA

Methods

All electronic medical record intraocular pressure (IOP) measurements were organized by laterality and date and associated with cataract surgery events (CPT 66982 or 66984). Analysis was limited to right eyes.  The number of days between the IOP measurement and the date of cataract surgery was calculated. Measurements were categorized in phases as: pre-op (up to 1 day before surgery), day of surgery, post-op day one, post-op less than 30 days after surgery, or post-op over 30 days after surgery. Patients were characterized as having a glaucoma diagnosis if they had on or more glaucoma billing diagnoses for any clinical service during the study period.

Results

Over 7½ years IOPs for 23,321 cataract surgeries were categorized as having no glaucoma diagnosis (baseline group, n=16,160), a glaucoma diagnosis with cataract surgery alone (glaucoma group A, n=5,523), or a glaucoma diagnosis with one of the following procedures performed concomitant with cataract surgery: stent (glaucoma group B, n=983), canaloplasty (glaucoma group C, n=456), trabeculectomy (glaucoma group D, n=136), or tube shunt surgery (glaucoma group E, n=63).  The mean pre-op IOPs were baseline 14.8 mmHg, group A 16.5, group B 17.0, group C 17.5, group D 18.9, and group E 22.4.  The mean changes in IOP >30 days post-op versus pre-op were baseline -1.9 mmHg, group A -2.4, group B -3.0, group C -3.3, group D -7.1, and group E -8.1.

Conclusions

Real world big data shows that glaucoma patients undergoing cataract surgery had higher pre-op IOPs and larger average decreases in intraocular pressure >30 days post-op versus pre-op both with and without the presence of a concomitant glaucoma procedure compared to non-glaucoma patients undergoing cataract surgery alone.
