Purpose

To report the first real-world results for the OMNI surgical system (Sight Sciences, Menlo Park, CA, USA) from the International Glaucoma Surgery Registry (IGSR) performed with phacoemulsification (Phaco) in patients with primary open-angle glaucoma (POAG).

Setting

Data from 10 independent centres in the United Kingdom.

Methods

Data extracted from the IGSR. This retrospective, multicentre, real-world study included eyes with glaucoma that underwent ab interno canaloplasty (CP), trabeculotomy (TR) or both (CP + TR) (combined or standalone). Efficacy outcomes included intraocular pressure (IOP), number of topical glaucoma medication classes up to 12 months postoperatively. Safety was also evaluated.

Results

This study included 403 eyes of 335 patients.

In the CP + Phaco group (n=39), mean IOP and classes of medications at baseline was 18.2 mmHg and 2.0. After 1 year, they were reduced to 15.6 (-2.6) mmHg and 1.8 (-0.2), respectively.

In the TR + Phaco group (n=163), mean IOP and classes of medications at baseline was 17.9 mmHg and 1.5. After 1 year, they were reduced to 13.3 (-4.6) mmHg and 1.4 (-0.1), respectively.

In the CP + TR + Phaco group (n=200), mean IOP and classes of medications at baseline was 17.5 mmHg and 2.5. After 1 year, they were reduced to 15.4 (-2.1) mmHg and 1.7 (-0.8), respectively.

Complications were infrequent and mostly self-limiting. The most common complications were hyphaema (6.7%) and transient IOP over 21 mmHg (3.0%).

Conclusions

CP, TR, or CP + TR reduced IOP and topical medication use in patients with POAG when performed in combination with phacoemulsification.  These real-world outcomes extracted from the IGSR affirm the safety and efficacy of OMNI surgery in real clinical practice in the UK.
