Purpose

comparative study of efficacy of standalone PreserFlo MicroShunt procedure versus
Phacoemulsification + Preserflo Microshunt in uncontrolled POAG in of Saudi Arabia

Setting

This prospective, interventional, nonrandomized case series was approved by the Institutional Review Board of Magrabi Eye Hospital, Jeddah, Kingdom of Saudi Arabia in the period between January 2024 to January 2025

Methods

This study included 39 eyes with uncontrolled POAG. Patients classified into group I
which included 21 eyes underwent combined Preserflo Micro Shunt +
phacoemulsification and group 2 which included 18 eyes underwent standalone
PreserFlo Micro Shunt procedure. All cases were followed up on day 1, day 7, day 30,
day 90 and day 180 after surgery

Results

  • • There is a trend toward a lower IOP in the PFM only group all over the follow-up
period, reached statistical significance only on the 1st postoperative day.
    • In the combined group, the is an early rise in the IOP (1 versus 3 months)
    • the success rate for both groups more than 85%.
    • The overall distribution of outcomes was similar between the groups, with no
    statistically significant difference found
    • The survival curves are nearly identical, with no statistically significant
    difference found between the groups (Log-Rank p = 0.91).
  • The KAPLAN MAYER  analysis showed no statistically significant difference in
the probability of remaining medication-free over the 180-day follow-up period

Conclusions

The minimum time needed to reintroduce anti-glaucoma meds was earlier in the
combined group (30 days) which is likely reflecting transient postoperative
inflammation from phacoemulsification while in the PFM only group, the late rise in
IOP is due to gradual tissue remodeling (encapsulation) around the tube
Very similar success rates throughout the 6-month which is more likely to maintain
comparable efficacy over ti
