Purpose

•The primary objective:

is to characterize the changes in corneal epithelial thickness profiles in patients with moderate dry eye disease and to correlate these findings with an increase in corneal higher-order aberrations.

•The secondary objective:

is to evaluate the therapeutic efficacy of topical sodium hyaluronate over a six-month period by measuring the potential reversal of epithelial irregularities and the corresponding reduction in higher-order aberrations.

Setting

Watany Eye Hospital and Assaf Clinic

Methods

Prospective observational study that included 28 eyes in patients with age group 20 - 40 years. Both males and females with mild to moderate dry eye disease NIBUT between (5 - 10 seconds)  were included. Patient with corneal opacities or dystrophies, previous intraocular or refrative surgeries, history of any ocular surface disorder other than dry eye, patients with history of irradiation or occupational assosciated dry eye were excluded. All patients were examined at day 0,90,180. All patients were instructed to use sodium hyalouronate 0.2% TID (three times/day). NIBUT, FBUT, lipid interferometry, meibography, tear meniscus height were recorded by the . Epithelial thickness map and wavefront maps were also recorded to detect HOA

Results

In terms of CDVA (logMAR), a significant difference was observed between the pre and post treatment measurements. NIBUT and FBUT showed significant improvement between pre and post treatment measurements. Lipid Interferometry also showed a significant shift, with all patients in the 60-80 nm category post treatment. However, no significant difference was observed in tear Meniscus Height or meibography. Superior, inferior, nasal Epithelial Thickness, and temporal Epithelial Thickness, showed significant differences were found between the pre and post treatment measurements.

In Coma Aberration, Trefoil Aberration, and Spherical Aberration, significant differences were found between the pre and post treatment measurements.

Conclusions

•Our study revealed significant improvements in CDVA, NIBUT, FBUT, and lipid interferometry after treatment.
• No significant changes were observed in tear meniscus height, meibography.
- Corneal epithelial thickness superior, inferior, nasal Epithelial Thickness, and temporal Epithelial Thickness, showed significant differences and regularization were found between the pre and post treatment measurements
•HOA  including coma, trefoil, and spherical aberrations, demonstrated significant alterations post-treatment, reflecting the influence of dry eye on visual quality.
•Correlation analysis highlighted the interrelationship between tear film stability, corneal epithelial thickness, and higher-order aberrations.
