Purpose
To evaluate the safety and efficacy of a newer generation combined bipolar radiofrequency (RF) and intense pulsed light (IPL) platform in patients with moderate to severe Meibomian Gland Dysfunction (MGD) using objective clinical parameters and validated patient reported questionnaires.
Setting
Retrospective Clinical study in tertiary eye care centre
Methods
This study included 30 patients with moderate to severe MGD refractory to conventional therapy.
All patients underwent three sessions of combined IPL and bipolar Rf therapy at 3 weeks interval. IPL was applied to the periocular skin to target telengiectasia and inflammatory mediators, and to enhance meibum liquefaction and peri-glandular tissue remodeling.
Assessments were performed at baseline and 12 weeks post treatment.
Subjective evaluation : Ocular Surface Disease Index (OSDI)
Standard Patient Dry eye questionnaire (symptom severity scoring)
Objective evaluation with tear Break up time (TBUT), Meibum quality and expressibilty Grading , Corneal Fluorescein staining
Results
Mean age was 49±10.4 years
Mean OSDI improved significantly from 46.2 ±8.1 at baseline to 20.9 ±6.7 at 12 weeks. TBUT improved from 4.9±1.5 seconds to 9.5±1.6 seconds.
Meibum quality improved by ≥ 1 grade in 80 % patients. Gland expressibility improved significantly .
Lid margin vascularity reduced significantly. Mild transient eryhtema occurred in 14% of patients and resolved within 24 hours. .No serious adverse effects were observed
Conclusions
New generation combined bipolar RF and IPL therapy is a safe and effective treatment for moderate to severe MGD. The synergistic mechanism of vascular modulation and controlled thermal remodeling results in significant improvement in both subjective symptoms and objective gland function.
