Purpose
To evaluate changes in the iridocorneal angle and anterior chamber depth (ACD) following deep anterior lamellar keratoplasty (DALK).
Setting
This retrospective study analyzed the postoperative outcomes of patients with keratoconus who underwent DALK at the tertiary referral center of “Magna Graecia” University of Catanzaro (Catanzaro, Italy), between February 2012 and June 2022.
Methods
The primary outcomes were iridocorneal angle width and ACD, measured using the 2D analysis module of anterior segment optical coherence tomography (AS-OCT) (CASIA SS-1000; Tomey, Tokyo, Japan). Measurements were collected preoperatively and at 1 day, 1 month, 6 months, and 1 year postoperatively.
Results
Overall, 124 eyes from 105 patients were included. The mean preoperative iridocorneal angle was 42.06±6.90 degrees. Angle width was significantly reduced at all postoperative time points (1 day: 37.31±6.69 degrees; 1 month: 41.63±6.41 degrees; 6 months: 41.43±6.11 degrees; 1 year: 41.55±6.16 degrees; all P<0.01).
Mean preoperative ACD was 3.52±0.54 mm and showed a significant reduction postoperatively (1 day: 2.24±0.49 mm; 1 month: 2.28±0.44 mm; 6 months: 2.37±0.42 mm; 1 year: 2.47±0.45 mm; all P<0.01). A statistically significant negative correlation was observed between best-spectacle corrected visual acuity at 1 year (0.14±0.16 logMAR) and ACD measured at 1 day postoperatively (ρ=−0.23, P=0.012).
Conclusions
At 1 year after DALK, a reduction of 0.51 degrees in iridocorneal angle and a decrease of 1.05 mm in ACD were observed compared with baseline. Greater early postoperative ACD was associated with better visual outcomes at 1 year.
