Purpose

To evaluate visual, refractive, and topographic outcomes of topography-guided epithelial and customized ablation (TREK and TCAT) combined with corneal crosslinking (CXL) in keratoconus, using a novel predictive software (FORESIGHT) that preoperatively simulates post-treatment corneal topography. The study investigates whether predictive planning translates into superior visual rehabilitation and short-term corneal stability.

Setting

Prospective interventional study, Tertiary Eye care Hospital

Methods

This prospective interventional study included 50 keratoconic eyes treated with TREK or TCAT for corneal surface regularization, followed by standard CXL, using the SCHWIND Amaris 1050RS excimer laser. The Foresight software was used preoperatively to predict post-ablation topography. UCVA, BCVA, refraction, keratometry, corneal symmetry indices, endothelial cell count, and disease progression were evaluated preoperatively and at 1, 3, and 6 months.

Results

At 2 months, eyes demonstrated statistically significant improvement in UCVA and BCVA with meaningful reduction in spherical and cylindrical refractive error. Keratometric analysis showed flattening of steep K values, reduced asymmetry, and improved corneal regularity. No endothelial compromise or progression was observed, indicating effective visual rehabilitation with maintained corneal stability.
6-month results awaited.
