Purpose

Corneoscleral contact lens is indicated for irregular astigmatism, including keratoconus. Keratoconus is a noninflammatory, progressive corneal thinning that can be highly asymmetric, affecting one eye clinically. Corneal crosslinking (CXL) is the first-line treatment of keratoconus involving riboflavin and ultraviolet irradiation to form new covalent bonds within the corneal extracellular matrix. As CXL merely slows the disease progression, corneoscleral contact lens contributes significantly to improve visual function. This case illustrates an asymmetric keratoconus patient who was prescribed corneoscleral contact lens 6 months after CXL procedure.

Setting

Cicendo National Eye Hospital, Bandung, Indonesia.

Methods

An 18-year-old male with keratoconus in the right eye and keratoconus forme fruste in the left eye presented for follow-up visit after underwent CXL procedure in the right eye. CXL was performed using accelerated epi-off mode for sixteen minutes and isotonic riboflavin with dextran was administered every three minutes. The UV-A treatment power was 5.6 mW/cm2 with total energy of 5,376 J/cm2. Best corrected visual acuity (BCVA) with spectacle correction was 0.16 in the right eye, whereas uncorrected visual acuity (UCVA) in the left eye was 1.0.

Results

Corneoscleral contact lens with power -18,25D, base curve 6,20 mm, and diameter 14,60 mm was prescribed for the patient, resulting in BCVA 0.5 after overrefraction. Follow-up visit was scheduled within six months to observe the change in the fellow eye.

Conclusions

Corneoscleral contact lens is preferred to correct and mask irregular corneal astigmatism after CXL procedure which halts the disease progression.
