Purpose

To evaluate the correlation between intended graft thickness based on femtosecond laser parameters and the actual thickness measured by anterior segment optical coherence tomography (AS-OCT) in patients undergoing femtosecond-assisted DSAEK.

Setting

46 patients underwent femtosecond-assisted DSAEK at Hull Royal Infirmary.

Methods

This retroprospective observational study included 46 eyes undergoing femtosecond-assisted DSAEK. Donor grafts were prepared using a femtosecond laser targeting a specified thickness. Postoperative graft thickness was measured using AS-OCT. The primary outcome was the difference between femtosecond-predicted and OCT measured thickness.

Results

The mean intended graft thickness using femtosecond laser settings was 145.3 μm, while postoperative OCT- measured thickness was 80.6 μm, representing a mean difference of 64.7 μm (approximately 45% thinner than intended).

Conclusions

Femtosecond-assisted DSAEK allows donor graft preparation however the thickness is significantly less measured post-operatively than predicted.
