Purpose

To report the clinical experience with topical losartan for treating series of cases with severe corneal haze after treatment of different Types of corneal ulcers

Setting

Clinical, retrospective, single center, non-randomized case series

Methods

Series of cases suffering from corneal haze after treatment of different types of corneal ulcers & infectious keratitis. After treatment of corneal Ulcers , Anterior segment optical coherence tomography confirmed the presence of a superficial anterior stromal central corneal opacity. Visual acuity & haze density were assessed with regular follow up .Patients began treatment of topical losartan 0.8 mg/mL six times per day that was tapered and monitored throughout 3 to 6 months

Results

Significant improvement in corneal opacity was observed at 1 to 2 months with almost complete resolution in some cases. Visual acuity improved from 1to 3 lines on the Snellen chart and remained stable.A significant reduction of corneal haze was observed at the slit lamp and using anterior segment optical coherence tomography

Conclusions

Topical losartan has anti-fibrotic activity through inhibition of the transforming growth factor-beta (TGF-β) signaling pathway. Topical treatment in conjunction with topical steroids is promising to treat corneal haze formation after different types of corneal Ulcers.Further clinical studies are needed to optimize losartan dosages and treatment durations
