Purpose

Mirvetuximab Soravtansine (MIRV), novel potential anticancer drugs of antibody-drug conjugates (ADCs) , used   for   ovarian cancer. However, their use is associated with ocular toxicities, including keratopathy, blurred vision, and dry eye.

This review summarizes current evidence on the incidence, and clinical presentation of MIRV-related ocular adverse events . It also provides practical, evidence-based strategies for the monitoring, and management of these adverse events

Setting

Retrospective observational study . Data were extracted from  the  population , undergoing a clinical trials with Mirvetuximab soravtansine in a reference centre in Madrid between 2021–2024 (30 months)

Report of case

10 female patients with advanced ovarian cancer,Full ophthalmologic examination at each treatment cycle

8   patients  with punctate keratitis ,  microcyst-like epithelial keratopathy  grad II

3   patients  with  catarat progression .

The major Events are CTCAE grade II in   both   eyes  . The median time to onset is 8-10 weeks. However, ocular toxicity may appear up to 4 months after treatment initiation, indicating a need for prolonged vigilance.

Treatment  includes prophylactic topical therapies, supportive care measures, treatment delays or dose modifications

Conclusion/Take home message

Mirvetuximab Soravtansine (MIRV) realated with high risk of ocular toxicity,

New ocular toxicity signals not documented in product labeling were detected.Further research is essential to measure the real outcome of preventive strategies and it will be necessary to validate our results in the future.
