Purpose

To report a surgical approach for acute hydrops and highlight the importance of regular ophthalmological follow-up for early keratoconus diagnosis.

Setting

The case was conducted in Médicos de Olhos SA, Curitiba, Brazil

Report of case

LHB, a 22-year-old male with no comorbidities or prior surgeries, has a family history of keratoconus. He was admitted to our service through the Unified Health System (SUS) complaining of itching and low visual acuity in the left eye (OS) for 2 days. Visual acuity: 20/100 in the right eye (OD) and Hand Motion in OS. Biomicroscopy showed hydrops in OS, and treatment with topical dimethyl polysiloxane, prednisolone (10 mg/mL) and carbonic anhydrase inhibitor (20mg/mL) was initiated. However, no improvement was seen after 1 month. Topography revealed snowman-type keratoconus in OD and no image capture in OS due to significant corneal irregularity.
A compressive suture was performed at the hydrops site in OS. Postoperative follow-up showed improvement in corneal edema and visual acuity to 20/200. The patient continues to be monitored.

Conclusion/Take home message

Keratoconus is a progressive corneal ectasia causing thinning and conical protrusion. In advanced cases, hydrops is an acute complication involving Descemet's membrane rupture, leading to corneal edema. Prompt identification and surgical intervention with compressive suture facilitate rapid corneal edema resolution and may prevent neovascularization. Regular ophthalmological follow-up is essential for early diagnosis and clinical management, preventing visual sequelae and quality of life impairment.
