Purpose
To present a case of penetrating keratoplasty with a sandwich tecnique.
Setting
This is a tecnique to eliminate open-sky time, in patients with high risk of elevated posterior pressure, in order to decrease chance of expulsion of intraocular contents.
Report of case
A 45-year-old female patient with a history of corneal perforation in the left eye (light perception) and corneal transplant for keratoconus in the right eye, currently opaque (20/400), presented with a history of pulmonary thromboembolism. General anesthesia was not authorized for this case. A penetrating sandwich keratoplasty technique was chosen to minimize open-sky surgery time, as the patient was under sedation and at high risk of postoperative hemorrhage due to her young age.
Conclusion/Take home message
Sanwich tecnique is useful tecnique in cases of increased posterior pressure.
This tecnique limits open-sky time during surgery.
