Purpose
To identify the association between corneal ectasia and orbital dermoid cyst on a case of unilateral corneal ectasia.
Setting
Memorial Institute for ophthalmic research
Report of case
A 25 year old female patient came complaining of blurring of vision in left eye for imprecise period, on examination Rt eye was completely normal with UCVA 1.0 while LT eye showed UCVA of 0.1 and BCVA of 0.3 with signs of CORNEAL ECTASIA and INFERIOR DYSTOPIA, IOP and fundus examination were normal on both eyes as well as ocular motility was completely free bilaterally.
pentacam was ordered showing completely normal Rt eye while grade 3 KERATOCONUS in Lt eye, then CT orbit was ordered that came to show Lt ORBITAL ANGULAR DERMOID CYST.
To the best of our knowledge there was only 2 previously documented case reports one with Orbital dermoid cyst combined with unilateral keratoconus published in October 2021 in the Indian Journal of Ophthalmology while the other for Isolated orbital fibrous dysplasia associated with ipsilateral keratoconus published in June 2010 in the American journal of ophthalmology case reports.
Conclusion/Take home message
Unilateral Corneal Ectasia can occur with space occupying orbital lesions mostly either due to ocular compression that increase the activity of inflammatory mediators, which lead to the biochemical changes and stromal thinning in keratoconus or "occasional eye rubbing" combined with the prolonged impact of dermoid cyst applied on the cornea resulted in a permanent thinning, tissue resorption and deformation of the cornea.
