Purpose
To demonstrate the surgical technique and clinical outcome of compressive corneal sutures in the management of acute corneal hydrops in a pediatric patient, highlighting its role in accelerating corneal edema resolution and improving visual rehabilitation.
Setting
Tertiary ophthalmic care center / cornea service at the Memorial Institute for Ophthalmic Research.
Report of case
We present a pediatric case of advanced keratoconus complicated by acute corneal hydrops. The child presented with sudden onset corneal edema, decreased vision, and discomfort. After clinical evaluation, compressive sutures were applied across the area of Descemet’s membrane break under general anesthesia. The video illustrates the step-by-step surgical technique, including suture placement, depth control, and postoperative management. Postoperatively, the patient showed rapid reduction of corneal edema, improved corneal clarity, and stabilization of the corneal surface, facilitating further visual rehabilitation.
Conclusion/Take home message
Compressive suturing is a safe, effective, and minimally invasive option for managing acute corneal hydrops in pediatric patients. Early surgical intervention can significantly shorten disease duration, reduce complications such as scarring, and improve visual outcomes. This video emphasizes the importance of timely decision-making and proper surgical technique in pediatric corneal emergencies.
