Purpose

To analyze the postoperative visual outcome of patients with macular pathologies after implantation of a toric intraocular lens (IOL).

Setting

This retrospective, monocentric study investigated patients who underwent toric IOL implantation between January 2024 and April 2025 at the Department of Ophthalmolog, at Kepler University Clinic in Linz, Austria.

Methods

Patients with macular pathologies were analyzed. Patients without any macular pathologies were used as a control group. Spectral Domain Optical Coherence Tomography (SD-OCT, Spectralis HRA+OCT, Heidelberg Engineering, Germany) was used for preoperative screening for macular diseases. Measurements were taken preoperatively and four weeks postoperatively using Biometry (IOL Master 700, Zeiss, Germany), Anterior Segment OCT (Casia 2, Tomey, Germany), autorefractometer (AR-1s, Oculus/Nidek, Germany) and subjective refraction.

Results

In total, 220 eyes from 152 patients (mean age 72,56 ± 11,51 SD) were included. Twenty-two patients (31 eyes) showed macular pathologies.. Mean preoperative visual acuity in the macular pathology group was 0.22 logMAR. Mean postoperative visual acuity was 0.13 logMAR.

Conclusions

The results suggest superiority of implanting toric IOLs in patients with astigmatism and macular disorder compared to monofocal IOLs.  Therefore, toric IOLs are justified in patients with macular pathologies for correcting the additional refractive error caused by astigmatism.
