Purpose

To assess rotational stability and refractive outcomes of a toric hydrophobic acrylic intraocular lens (TIOL) with double C-loop haptics.

Setting

Prospective, single center, clinical study at the Medical University of Vienna

Methods

A total of 41 eyes of 26 patients with age-related cataract and total corneal astigmatism of greater than 1.0 diopters (D) received a hydrophobic acrylic toric PODEYE IOL (BVI Medical, Inc, USA). Baseline measurement for rotational stability evaluation was performed at the end of surgery (EOS), with the patient still supine on the operating table, using non-movable vessels as reference landmarks. Postoperative retroillumination pictures were taken at 1 h, 1 week, 1 month and 4-6 months postoperatively. In case of severe rotation necessitating secondary re-rotation of the TIOL, the last observation was carried forward to the overall result. Subjective manifest refraction was assessed at the 6 months follow-up visit.

Results

Mean absolute rotation from EOS to 6 months was 1.44 ± 2.03 [0.09;24.51] degrees. Rotational stability values from EOS to 1 h, 1 h to 1 week, 1 week to 1 month and 1 month to 6 months were 1.86 ± 4.06 [0.02;24.51], 0.36 ± 0.36 [0.01;1.57], 0.34 ± 0.32 [0.01;1.35] and 0.32 ± 0.27 [0.00;1.38] degrees. Mean preoperative corneal astigmatism was 1.53 ± 0.64 [1.03;4.17] Diopters (D) and changed to a mean postoperative refractive astigmatism of 0.18 ± 0.20 [0.00;0,75] D at 6 months.  ThreeTIOLs exhibited rotation of greater than 5° between EOS and 6 months postoperatively. One TIOL with 24.51° of rotation between EOS and 1h was rerotated 2 weeks after the primary surgery.

Conclusions

The PODEYE toric IOL demonstrated good rotational stability, with a mean absolute rotation of 1.4° between the end of surgery (EOS) and 6 months postoperatively. The majority of TIOL rotation occurred between EOS and 1 hour postoperatively, with one TIOL rotating more than 20°, requiring repositioning surgery. Refractive outcomes were satisfactory, with a mean residual refractive astigmatism of less than 0.50 D.
