Purpose

To evaluate the angular misalignment at 3 months postoperatively relative to the intended implantation axis of the Asqelio Trifocal Toric intraocular lens (AST VisionCare).

Setting

Hospital Arruzafa, Córdoba, Spain.

Methods

Prospective study included eyes implanted with the Asqelio Trifocal Toric IOL. Preoperative biometric assessment and toric IOL planning were performed as part of routine clinical practice. Intraoperative IOL alignment was achieved using the Callisto digital image-guided system, and the implantation axis was recorded as the intended axis. At 3 months postoperatively, manifest refraction was obtained and IOL axis orientation was assessed using Pentacam retroillumination. Angular misalignment was defined as the minimal absolute angular difference between the intended intraoperative axis and the axis measured at 3 months. As immediate postoperative axis measurements were not available, true postoperative rotation could not be directly assessed.

Results

47 patients, 22 men and 24 women. Retroillumination-based measurements of IOL axis orientation at 3 months were available in 51 eyes. Mean absolute angular misalignment was 4.4±4.0°, with a median value of 3.0°, indicating a generally small deviation from the intended implantation axis. Approximately half of the eyes exhibited an angular misalignment of ≤3°, while most remained within a low misalignment range. A limited number of eyes showed higher angular deviations, contributing to the observed variability. Mean residual refractive cylinder at 3 months was -0.14±0.29 D. A total of 91.2% of eyes achieved a residual refractive cylinder of ≤0.50 D, and 98.5% achieved ≤1.00 D, consistent with the low angular misalignment observed at 3 months

Conclusions

Asqelio Trifocal Toric IOL demonstrated a low degree of angular misalignment from intended implantation axis at 3 months postoperatively. Although true rotational stability cannot be inferred due to the lack of immediate postoperative measurements, the limited angular misalignment observed was associated with excellent refractive astigmatic outcomes.
