Purpose

To evaluate the feasibility and clinical performance of a new customized toric asymmetrical intraocular lens (IOL) concept for the correction of irregular corneal astigmatism in cataract surgery. In addition, the consistency between different diagnostic platforms (CASIA2, MS-39, Pentacam, ANTERION, IOLMaster 700) in cases of irregular astigmatism will be investigated.

Setting

Department of Ophthalmology, Johannes Kepler University and Kepler University Clinic Linz, Austria

Methods

This prospective, monocentric pilot study (planned n=15) includes cataract patients with total corneal astigmatism ≥1.75 D and ≥0.75 D of asymmetry or higher-order irregularity within the 6-mm zone of the total corneal refractive map (CASIA2). Patients undergo standard phacoemulsification with in-the-bag implantation of an individualized toric asymmetric IOL (AMILens Individual). Examinations are performed preoperatively and at 1 and 4–8 weeks postoperatively, including UDVA, BCDVA, slit-lamp examination, and VA-OCT imaging (CASIA2, MS-39, Pentacam, ANTERION, IOLMaster 700). The primary endpoint is the change in total corneal astigmatism from baseline to 4–8 weeks postoperatively.

Results

So far, five patients have been recruited. However, no postoperative data are available at the moment of submission. Of the recruited patients, two have stable keratoconus, two are status post PKP, and one has irregular astigmatism after pterygium excision. The mean preoperative UDVA is 0.4 logMAR, and the mean BCDVA is 0.2 logMAR. The mean total preoperative astigmatism is 4.75 dpt. The mean asymmetry, based on the Fourier analysis of CASIA 2 data, is 1.99dpt in the 3mm zone and 2.87dpt in the 6mm zone.  Detailed postoperative results including UDVA and BCDVA of the complete cohort will be presented in September at the ESCRS Congress.

Conclusions

We assume that the implantation of a customized toric asymmetric IOL in eyes with irregular corneal astigmatism leads to a clinically relevant reduction in astigmatism and a functional improvement in uncorrected visual acuity. The complete data will be presented at the ESCRS Congress in September.
