Purpose
The purpose of the study was to compare the efficacy and safety in astigmatism correction and the postoperative visual outcomes between the implantation of a trifocal toric IOL and trifocal IOL implantation with limbal relaxing incision (LRI) in patients with moderate corneal astigmatism seeking for presbyopia correction, and to evaluate the long-term rotational stability of the toric IOL.
Setting
single-center retrospective observational case study
Methods
This study enrolled patients with corneal astigmatism 1.0–2.0D, including 25 eyes of 15 patients who underwent refractive lens exchange with trifocal toric IOL implantation and 25 eyes of 16 patients who underwent trifocal IOL implantation with LRI. Postoperative evaluations at 1 and 6 months included near, intermediate, and distance UDVA, BDVA, manifest refraction, and defocus curves. Toric IOL axis position was assessed via slit lamp at 1 day, 1 week, 3 and 6 months postoperatively and compared with the preoperative planned position. Subjective satisfaction was evaluated using questionnaires on spectacle dependence, vision quality, and overall satisfaction. Outcomes and adverse events were compared between these two group.
Results
At 3 months postoperatively, in trifocal toric IOL group, CDVA, UDVA, UNVA and UIVA were -0.05±0.07, -0.01±0.07, 0.09±0.02, and 0.10±0.01 logMAR, respectively; in the trifocal+LRI group, the values were 0.08±0.10, 0.09±0.07, 0.1±0.08, and 0.14±0.07 logMAR, respectively. SE of manifest refraction was 0.12±0.17D (trifocal toric IOL group) and 0.25±0.35D (trifocal+LRI group), with mean refractive cylinder of 0.14±0.16D and 0.34±0.42D, respectively. Defocus curves showed a smooth range of good visual acuity. The mean toric IOL axis deviation at 3 months was 2.9±2.4° (range: 1°-19°), with 91% of eyes showing deviation <5°. Subjective scores for spectacle dependence, vision quality, and satisfaction did not differ significantly between groups.
Conclusions
Implantation of a diffractive trifocal toric IOL for presbyopia, and refractive error correction provided good refractive and visual outcomes, relatively smooth range of intermediate vision, and high levels of visual quality and patient satisfaction until 6 months after surgery. Compare to LRI, trifocal toric IOL provided a relatively better BCVA, UCVA, and intermediate vision. However, the rotational stability needs to be observed in long term.
