Purpose

Visual outcomes of toric intraocular lenses (IOLs) are highly impacted by lens rotation.  The purpose of this study is to evaluate the impact of geometry on the forces acting on IOL haptics in Alcon ISO 11979 Standard Compression Tester computational model to infer IOL rotational stability.

Setting

Compression testing conducted at Alcon Research, LLC, Fort Worth, Texas, USA.

Methods

In this study, the impact of variability in IOL geometry on the forces acting on the IOL along and orthogonal to the haptic compression direction was evaluated in the Alcon Compression Tester computational model as the haptics are compressed from their undeformed state 9 mm compression diameter. Acrysof® SA6ATx and Clareon® CCW0Tx (Alcon Vision), Tecnis® Toric IOL (JnJ), Vivinex® Toric XY1A-SP (Hoya Optics), enVista® Toric IOL (B+L), and Santen Toric IOL (Santen) were studied.

Results

Forces acting on the IOL haptics ranged from 0.13 to 0.69 mN along the haptic compression direction, 0.003 to 0.33 mN along the optical axis, and 0.08 to 1.45 mN along the circumferential direction for various IOLs as the haptics compressed from 9 to 10 mm.

Conclusions

The circumferential forces acting on the Clareon IOL haptics are lowest among the IOLs studied, indicating low tendency for IOL rotation. A low positive slope for the circumferential force differential with compression diameter was observed, indicating greater rotational stability of the Clareon IOL in smaller capsules.
