Purpose

To evaluate long-term safety, refractive predictability, and visual efficacy of individually calculated toric intraocular lenses (IOLs) implanted in cataract/ RLE patients with extreme refractive errors, using real-world data:

Setting

All surgeries were / will be performed at the Breyer-Kaymak-Klabe Eye Surgery and Premium Eyes in Duesseldorf, Germany, member of the International Vision Correction Research Center (IVCRC.net) by one single surgeon (Breyer).

Methods

Retrospective analysis of 234 eyes undergoing toric IOL (LENITS Mplus LU-313 MF15T, Teleon Surgical GmbH) implantation, focusing on patients with high astigmatism (from -0.50 D to -7.50 D) or extreme refractive errors (from 8.00 D to -23.25 D). Eyes were excluded  if ocular comorbidities affecting visual acuity were present (n=60). Safety and predictability analyses included all 174 eyes. Efficacy analysis was performed in subgroups based on available follow-up: 1 week–5 years (n=112), 3 months–5 years (n=65), and eyes with UDVA ≥0.8 decimal at ≥3 months (n=56).

Results

Low CDVA line loss indicated favorable safety. High refractive predictability with most eyes within ±0.50 D of target, even in eyes with extreme baseline refractive errors. UDVA remained stable across analyzed follow-ups, with eyes achieving UDVA ≥0.8 decimal maintaining consistently high visual performance.

Conclusions

Individually calculated toric IOLs are safe and effective in patients with extreme refractive errors, providing reliable refractive outcomes and sustained visual efficacy. Subgroup analyses highlight robust visual performance in eyes achieving target refraction with UDVA ≥0.8 decimal. These findings support the clinical use of customized toric IOL calculations in routine cataract surgery for complex refractive cases.
